What is Lichen Planus?
Lichen planus is a chronic mucocutaneous inflammatory disorder manifested across the skin and mucosal surfaces. And one of the commonly occurred autoimmune disorder like vitiligo, patchy alopecia (a. areata) or colitis (ulcerative), chronic urticaria etc. It is a pruritic disease affecting the skin, mucous membranes, and hair follicles. It causes swelling and irritation in the skin, mucous membranes, hair, and nails and characterized by small, purplish brown, itchy eruptions. The course of the Lichen Planus is unpredictable and spontaneous remission is also seen in some patients. It means that in some patients, it may disappear on its own. Commonly affected areas are the front of wrists, shins, trunk, medial thighs, legs, glans penis, etc. Some variations are more common in Asia, Latin America, the middle east, and Africa. It occurs at any age. Skin lesions occur equally in women and men, but women are more likely to develop oral lesions; it is infrequent in children and older people but widespread in middle-aged people.
What are the symptoms?
The medical background of lichen planus is not fully understood. Sometimes lichen may be associated with Hepatitis- C infections or other autoimmune disorders like SLE, psoriasis, diabetes, inflammatory bowel diseases, alopecia areata, vitiligo, primary biliary cirrhosis, etc. Lichen planus is regarded as an immune-mediated reaction pattern of the skin due to diverse causes; immunological reactions in lichen planus are mediated by T cells. - Genetics- Lichen planus is known to occur in families with a higher level of HLA-B7.

What are the symptoms?
A common symptom is sub-acute eruptions with intense itching. Apart from itching following symptoms are frequently observed in patients with Lichen Planus:
- Purplish eruptions or pinkish or greyish eruptions on the skin
- Flat-topped polygonal papule covered with adherent scales
- Itching at site of skin rash
- Blisters may burst and become scabby.
- White lines on the surface – Wickham’s striae.
- Oral and genital mucosa is involved with different lesions, including a white lacy pattern, tiny white papule, or ulcerative lesions. These are painful and cause a burning sensation.
- Nail changes can be seen as linear ridges, discolouration, dystrophic lesions, and pterygium formation.
- Less commonly, it affects the nails, mucous membranes, and scalp.
- Malignant changes occur in hardened longstanding ulcerative lesions, especially lesions of the oral mucosa that are more prone to malignant transformation.
What are the various types of lichen planus?

Linear Annular
Atrophic Hypertrophic
Eruptive Follicular
Lichen planus pigmentosus Vesicular or bullous
Erosive or ulcerative Actinic
Inverse.
How the lichen planus is diagnosed?
A proper clinical examination is mostly enough to diagnose the condition. Sometimes a biopsy may be taken. The diagnostic characteristics are:-
- Intense pruritus
- Erythematosus to violaceous colour for the early lesion
- Flat-topped polygonal papules
- Intense black pigmentation on healing
- Koebner phenomenon is present.
